Evolve Rehabilitation and Pilates provides physical therapy and Pilates instruction, and as such requires certain personal information in order to carry out these services. Information collected includes your name, how we can contact you and certain sensitive information regarding your health, medical history and date of birth.

What data we collect

When you first come to Evolve, we collect the following data:

• Your name;
• Your contact information, including email address, phone number and address, as well as emergency contact name and phone number;
• Personal health history: medications and health conditions, illnesses and surgeries.

We do not store your credit card information.

Why we collect your data

We collect your contact information in order to schedule or change appointments or relay any information you need regarding your scheduled activities.

We collect health information in order to better understand your physical condition and any movement restrictions which would require adapting or eliminating exercises.

How we protect your information

Evolve Rehabilitation and Pilates is committed to securing your data and keeping it confidential. Data we collect from you is stored locally only; it is not stored anywhere online. Contact information can be accessed only  by the one person responsible for scheduling and rescheduling  appointments. Your health history is accessed infrequently only by your instructor or Anthony for consultation.

Restricting the collection of your personal data

Evolve Rehabilitation and Pilates will not lease, sell or distribute your personal information to any third parties unless we have your permission or if we are required to do so by law.
